Rates and Fees
I am an out of network provider and do not file insurance claims. I will provide you with a receipt for your payment and/or a Super Bill, upon request, that you can turn in to your insurance company for reimbursement.
My rates and fees for services are very competitive and will be discussed when you call to schedule an appointment or request an appointment through the client portal. Client fees and rates will also be included in my forms that you will receive to complete prior to the first Intake session. An initial 30 or 60 minute office consultation can also be scheduled for more in-depth questions and discussion about your needs and my services.
Services may be covered in part or in full by your health insurance or employee benefits plan. Please check your coverage by asking the following questions:
- Do I have mental health (behavior health) insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment is due at the time of service before your session begins. I accept cash, Bank Debit Cards (with VISA or M/C logo), ACH Bank Transfer, VISA, MasterCard, Discover, and American Express. Payment for professional therapy services may also be processed through your Flexible Spending Account (FSA) and/or Health Savings Account (HSA).
Your therapy time is reserved for you and/or your family member. If you do not show up for your scheduled appointment and have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. Please cancel Monday appointments by 5:00 p.m. on Friday and appointments after holidays during the business day before.
